2019 Gaziano Offensive and Defensive Lineman Awards
Required Nominee Information:
In order to be nominted for one of the Awards, the following information
will be required no later than the date listed on the timeline:

In order to be nominted for one of the Awards, the following information
will be required no later than the date listed on the timeline:
- Completed Application Form
- A nomination letter from the Nominee's Coach designating wether the individual is being nominated for either the Offensive or Defensive Lineman Award
- A letter of recommendation from the Principal
- A letter of recommendation for the Athletic Director
- A current copy of the Nominee's transcript with class rank
- A copy of Nominee's SAT/ACT scores
- An essay from the Nominee
- Two game videos of the nominee playing against their teams two best opponents. If your school participates in HUDL, submission instructions for providing these highlight films to us may be found on our Evaluation Criteria Page.
- Photo of Nominee in uniform without helmet on.
All credentials completed and submitted by December 4, 2019 and mailed to:
